Submission of form and deposits is NOT confirmation of registration - an enrollment confirmation email will be sent once your spot is secured.     • 2023 SI Elevé Order Form

      We are extremely proud to partner with Elevé Dancewear, an internationally known company based right here in Kansas City. Founder/owner Lisa Choules is a former dancer with Kansas City Ballet.

      We have customized a leotard and uniform shirt just for the 2023 Kansas City Ballet School Summer Intensive. We are excited to offer a beautiful Juliette leotard in Sangria with velvet cap sleeves, embroidered with the KCBS logo. Male identifying dancers will receive a Gabe shirt in Sangria. One Juliette Leotard or Gabe shirt is included in your Summer Intensive Tuition. The leotards are made to order and total production time (with embroidery) is estimated at 6-8 weeks. This means the leotards/shirts may not be ready at the start of the Intensive. All orders must be received by April 1. Additional shirts and leotards may be ordered on the Summer Intensive Student Page.

      Do not contact Elevé Dancewear regarding your order. All orders must be placed through the Kansas City Ballet School.

    • Sizing Information

      Elevé leotard sizing is unique and designed for a dancer's long and lean body. The adult S and M are the same around but size M has a longer girth. Elevé can also do custom sizing, such as XS Long or S top/M bottom, but those leotards would not be exchangeable. Standard sizes can be exchanged after purchase if we happen to have any additional stock.

    • All sizes listed are adult sizing.
    • DORM STUDENT INFORMATION

    • Weekday lunches are prepared by the KCAI Dining Hall and are brought to KCB each day, where they are refrigerated. All other meals take place in the KCAI Dining Hall and students choose what to eat. Please indicate the type of weekday lunch the student would like to receive below.

      KCBS will provide KCAI with all lunch preferences and food allergies.

      KCBS must be notified of any lunch preference changes no later than May 19, 2023.

    • All dorm students receive one t-shirt as part of their housing package. Dorm students may purchase any additional t-shirts for $25. This form must be submitted by April 1.

      Shirt will be Bella + Canvas unisex jersey short sleeve tee. Design is currently in progress!

      Each student must request the other in order for the placement to be honored.

    • DORM PLEDGE

      Please read carefully before signing.
    • As a Kansas City Ballet School (KCBS) student in residence at the Kansas City Art Institute (KCAI) I realize that I share responsibility for the vitality and well-being of my community and understand that there are certain rules established to assist in maintaining that environment. I understand and agree to uphold these policies while I attend KCBS and reside on the campus of KCAI.

      Personal safety

      • When not at KCBS or attending an activity, students must remain in the dormitory at all times unless properly checked out for a walking excursion or release with parents/guardians/friends/relatives.

      • Students must abide by all rules for safe conduct at KCAI, KCBS and KCBS-related activities.

      Activities are required for all dorm students.

      Curfew must be followed by all dorm students. Curfew times are based on age.

      Cars driven by students to the program may be parked at KCAI and must be registered with the Dorm Director and KCAI Security. Students may not use the car during the program and the keys must remain with the Dorm Director.

      Pets are not allowed at KCAI, or KCBS.

      Physical contact between students is not permitted other than during Pas de Deux class.

      Theft and damage incurred by students are their responsibility. Students agree that they must respect the personal property of other students, faculty and staff of KCBS and KCAI during the program.

      Firearms, explosives and weapons are strictly forbidden at KCAI, KCBS and KCBS-related activities.

      Smoking and consumption of tobacco products are prohibited to minors in the State of Missouri and the use of cigarettes, e-cigarettes, cigars, pipe and chewing tobacco is forbidden at KCAI, KCBS and KCBS-related activities.

      Alcoholic beverages are prohibited to minors in the State of Missouri and the possession or consumption of alcoholic beverages (regardless of age) is forbidden at KCAI, KCBS and KCBS-related activities.

      Drugs, other then prescription medications or over-the-counter medications provided by parents/guardians, are strictly forbidden at KCAI, KCBS and KCBS-related activities.

      I understand I must accept responsibility for my own actions. If I am found to be in violation of the principles governing our community there will be disciplinary measures taken by the staff. I understand that the administration of KCBS and KCAI have the right to inspect my room and belongings if it is suspected or deemed apparent that I am in violation of any of the regulations. I understand that should my actions warrant, I may be required to leave the program, and my parents/ guardians will be responsible for expenses incurred in order for me to return home.

      I understand that violation of any rule of conduct may be grounds for instant dismissal from the program.

      As a parent/guardian, I understand the regulations and guidelines my student is expected to abide by, and the responsibility they incur in attending KCBS and residing at KCAI. I support such rules and understand my responsibility if my student does not abide by them. I will pay any and all expenses incurred to return my student to their home because of dismissal from the program. In such a situation, I understand no refund of tuition or fees will be forthcoming.

    • WAIVER AND RELEASE

      Please read carefully before signing. This is a release of liability and waiver of certain legal rights.
    • Liability Release: As the enrolled participant and/or the parent/guardian of the participant, I agree and understand that dance/fitness training is a potentially hazardous activity. I recognize that there are risks inherent in dance training, including but not limited to serious physical injury. The participant hereby agrees to participate in activities of the Kansas City Ballet School (KCBS) and hereby agrees to indemnify and hold harmless Kansas City Ballet(KCB), KCBS, and the instructors, officers, directors, agents and employees of each against any liability resulting from any injury that may occur to the participant while participating in activities of the KCBS. The participant also agrees to indemnify KCB and KCBS for any damages incurred arising from any claims, demand, action or course of action by the participant. .

      Medical Release: As the enrolled participant and/or the parent/guardian of the participant, I agree and understand that during the Summer Intensive program it may be necessary for the participant to receive immediate hospital and medical attention without the specific consent of the undersigned in order to protect the health and welfare of the participant and the other students. In view of the foregoing, it is agreed that when, in the opinion of the Kansas City Ballet School, and/or Kansas City Art Institute (KCAI), if the participant shall need hospital or medical care, or both, that KCBS, and/or KCAI shall have the power, and are hereby authorized to see that said student is hospitalized or receives medical care, or both. Further, the participant and/or parent/guardian agrees to pay all costs associated with medical care and transportation for the participant. Any special medical/health problems of which the staff should be aware are disclosed on the Health History form. The undersigned shall fully indemnify and hold harmless KCB, KCBS and KCAI, and their trustees, officers, agents and employees from any claim or liability resulting from its actions authorized here under.

      Photography/Videography Release:As the enrolled participant and/or the parent/guardian of the participant, I authorize Kansas City Ballet and/or its representative, agent or employee to photograph and/or videotape and use any photograph/likeness of me or the student for any purpose, including publicity, choreographic archives, promotional materials and/or any other reason deemed appropriate by the School Director. .

      Release to leave with parents/guardians/friends/relatives while attending KCBS: I hereby release and discharge KCB, KCBS, KCAI, and their trustees, officers, agents and employees from any and all responsibility, personal liability, losses, claims or damages arising out of, or in connection with, trips scheduled by the parents/guardians/friends/relatives of the student during the time enrolled in the KCBS programs or in residence at KCAI. Before a student can be released to parents/guardians/friends/relatives, there must be a signed release form on file with KCBS by the 9:00 PM on the Thursday prior.

      Field Trip/Walking Excursion Release: I hereby release and discharge KCB, KCBS, KCAI, and their trustees, officers, agents and employees from any and all responsibility, personal liability, losses, claims or damages arising out of, or in connection with any field trips or walking excursions during the time the student is enrolled in the KCBS programs or in residence at KCAI. All scheduled activities that are held away from KCAI will be supervised by the Dorm Director and Resident Assistants (RAs). Groups of students may take walking excursions from KCAI during designated times (e.g. to the Country Club Plaza, Nelson Atkins and Kemper Art Museums). Students must sign in and out with the Dorm Director or RA on duty and return to KCAI at the specified times for the end of the excursion.

      Staff Transportation Release: I hereby authorize KCBS staff (including Dorm Director and RAs) to transport the student in a vehicle to and/or from KCAI, KCB, medical appointments and/or other locations as necessary. I agree to fully indemnify and hold harmless KCB, KCBS, KCAI, and their trustees, officers, agents and employees from any claim or liability resulting from this transportation.

    • Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

      The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

      The Kansas City Ballet School (KCBS) has created Safety Guidelines and response protocols for our 2022-2023 Academic Year. The attached guidelines were prepared in consultation with medical experts, the Center for Disease Control and Prevention (CDC), and state and local officials on the opening of schools, as well as our own research in our particular field for operation, including in consultation with our colleagues from dance schools across the nation.

      Kansas City Ballet has put into place these preventive measures in an effort to reduce the likelihood of the spread of COVID-19; however, even if these procedures are strictly followed, KCBS cannot guarantee that you or your child (the “Student”) will not become infected by COVID-19.

      By signing this release, you acknowledge your child’s risk of being exposed to or infected by COVID-19 by attending any class held at the Bolender Center for Dance & Creativity or the Johnson County Campus of Kansas City Ballet School or by participating in any program of the Kansas City Ballet School (which programs, for purposes of this document, include residential living and social activities associated with KCBS Summer Intensive or other residential programs). This exposure may result from the actions, omissions, or negligence of Kansas City Ballet, Kansas City Ballet School, and others, including but not limited to, faculty, staff, and class participants and their families, or from circumstances which are unknown or beyond anyone’s control.

      READ CAREFULLY AS EXECUTION OF THIS FORM WILL WAIVE LEGAL RIGHTS:

      By signing below, you acknowledge that:

      I understand that KCBS will try to follow the Safety Guidelines referenced above, but I also understand that I cannot expect perfect compliance. I acknowledge and understand that, even if there is strict adherence to the procedures referenced above, there is still a risk that my child attending the Program can contract COVID-19 while on Kansas City Ballet School premises or participating in Kansas City Ballet School programs. I also acknowledge that due to their physical nature, dance programs may carry a greater risk than other activities of exposure to COVID-19. Even when all reasonable precautions are taken, there is an unavoidable risk of exposure to COVID-19.

      I knowingly and voluntarily agree to assume all the risks of COVID-19 and accept sole responsibility of any infection (including but not limited to personal injury, disability, and death) or expense of any kind, that I or my child may experience or incur arising from my child’s attendance at Kansas City Ballet School classes and workshops or use of Kansas City Ballet School facilities or other participation in Kansas City Ballet School programs.

      In consideration of the acceptance of Student for participation in a KCBS program, Student and his/her assignees, heirs, parents, guardians, and legal representatives hereby voluntarily release, forever discharge, and hold harmless Kansas City Ballet, Kansas City Ballet School, and each of their officers, trustees, employees, faculty, staff, students, agents, representatives, or groups affiliated therewith (collectively “the Kansas City Ballet Releases”) from any liability, claims, or causes of action of whatever nature, arising from or related to exposure to COVID-19 for any reason whatsoever to the fullest extent permitted by law, including without limitation claims based on the actions, omissions or negligence of any of the Kansas City Ballet Releases or claims arising out of Student’s attendance at Kansas City Ballet School classes and workshops or use of Kansas City Ballet School facilities or other participation in Kansas City Ballet School programs.

      I acknowledge by signing this release that I have read and understand the KCBS Safety Guidelines and Protocols. I also understand that my assumption of risk and release of claims applies, without limitation, to any COVID-19 infection (whether it occurs before, during, or after participation in classes or workshops or other programs). By signing this document, Student and his/her assignees, heirs, parents, guardians, and legal representatives waive the right to bring any legal action now or at any time in the future to recover compensation or obtain any other remedy for any injury to Student or Student’s property—including his/her death—caused by COVID-19 or COVID-19-related procedures and arising out of his/her participation in classes or workshops or use of Kansas City Ballet School facilities or participation in any program of the KCBS. I also understand that the Student will not be accepted into the program unless this document is signed with acknowledgement of understanding of the risks involved.

      In addition, I understand that this release is intended to be as broad and inclusive as the State of Missouri will allow and that if any portion is held invalid, agrees that the balance shall, not withstanding, continue in full force and effect.

      By signing, I acknowledge and represent that I am authorized to execute the Assumption of Risk and Waiver of Liability with reference to the Students whose name(s) I have listed below. I understand and agree that this Assumption of Risk and Waiver of Liability is applicable to all Students listed, below, and to every Kansas City Ballet School class or workshop or program attended by any such Student after the date I sign this document.
    • By printing my name below, I acknowledge that I have read the above releases and sign with the full knowledge of their content and significance. Both parties agree that a printed name is the legal equivalent of a manual/handwritten signature on this form.
    • PAYMENT INFORMATION

      Tuition: $2,900
      Housing Package: $3,200 (Includes dorms and cafeteria)


      A non-refundable $500 deposit is required to reserve a place in the program. A non-refundable $500 deposit is required to hold a place in the dormitory. These deposits are applied toward the total tuition/housing costs.

      The balance of fees is due by May 19, 2023. The program will be filled on a first-come, first-serve basis.

    • By selecting Self Pay, you agree to submit payment in full by the May 27 deadline. Payments may be made by mailing checks or by making payment over the phone by calling the Kansas City Ballet School at 816-931-2299.
    • Refund Policy

      Non Refundable Deposits - No exceptions

      - $500 housing deposit for dorm students
      - $500 tuition deposit for all students registered for the SI

      Deadlines
      - Final payments are due by May 19, 2023
      - Any accounts with a balance remaining on May 22, 2023 may be subject to a $150 late fee
      - Any accounts with a balance remaining on May 27, 2023 with no communication from the family or payment plan in place will forfeit their spot in the program.

      Injury
      In the event a student is injured to the point where they are unable to participate PRIOR to the beginning of the program:
      - A doctor’s note explaining the injury must be received by KCBS
      - Deposits are still nonrefundable
      - Payments made beyond deposits will be refunded via check.

      COVID
      In the event we are forced to shift to a virtual program due to COVID-19:
      - Students will be given the option of a virtual program. Deposits can be shifted to the cost of the virtual program ($1,000).
      - Any payments made beyond the cost of the virtual program will be refunded.
      - Deposits will still be nonrefundable

    • REGISTRATION CONFIRMATION

      Submission of the registration form is not confirmation of enrollment. Please allow up to ten (10) business days for KCBS staff to process enrollments. You will receive a confirmation email once your spot is confirmed.
