  • Upon submission of this form you will receive an order confirmation at the above email address. When the credit card is run or the payment is entered in our system, a receipt will be sent to the email address on the student's KCBS account.

  • Order Information

    KCBS Junior Summer Intensive T-shirts cost $35 each. This form must be submitted by April 7th.

    Shirt will be Bella + Canvas unisex jersey short sleeve tee. Design is currently in progress!

    Adult XS 31"-34"
    Adult S 34"-37"
    Adult M 38"-41"
    Adult L 42"-45"
    Adult XL 46"-49"

    Youth Large fits sizes 14-16.

  • If you would like more than one shirts, please indicate the following: shirt #2: size, shirt #3: size, etc.

  • Payment Information

  • If not selecting to charge card on file or add to total balance, payment must be received by April 7th or your shirt(s) will not be ordered.
  • To update the credit card on file, login to your Mindbody account or create a new account. Please make sure to update the credit card information on the account that matches the order name. Update card here.
